How to register online
As a new patient, you can now register online using a PC, smartphone, or tablet from the comfort of your home and at a time that suits you.
Simply click the button below. Please complete all the fields on the registration form – incomplete forms will delay registration.
Our catchment area covers all of E8, most of E5, and some parts of N1 and N16.
After you have registered online, you will be offered an appointment to attend the surgery to have a new patient health check with the practice nurse. Please fill in and bring with you the Health Questionnaire (or alternatively, this can be filled out when you attend).
You will also be able to book an appointment to see a GP (usually within 24-48 hours depending on the urgency of your problem).
How to register in person
Just download and complete the forms below.
Please bring the completed forms along to the reception with your medical card if you have it. The information on the medical card will speed up the process of getting your records from your previous practice. Once you have been accepted as a patient your medical records will be transferred to us and a new medical card will be sent to your home address.
When you register, we will ask you to make an appointment to see the practice nurse for a new patient health check. This will make sure that our records are accurate (e.g. what medications you’re currently taking, any significant medical history, etc) and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests) are up-to-date. Medical treatment will be available from the date of registration.

Updating your personal details
If any of your personal details, e.g. address or telephone number, change, please let us know. For your safety, it is vital that the information we hold is up to date and accurate.
Accountable / Named GP for all patients
All patients registered at the surgery have a named doctor who has overall responsibility for your care and support. You can, of course, choose to see another doctor at the surgery. Please let the reception team know if you would like to book an appointment with a specific doctor.
